Rays of Hope Community LLC is a DDD-certified, Medicaid-approved Day Habilitation provider serving adults 21 and older with intellectual and developmental disabilities (IDD) across Central and Northern New Jersey.
Day Habilitation is a structured, community-based program designed to help individuals with IDD build skills, develop independence, and engage meaningfully with their communities — all in a safe, supportive, and person-centered environment.
We understand that the transition from school-based services to adult services can feel overwhelming for families and support coordinators. We actively partner with school transition teams, Family Success Centers, and social service agencies to help families navigate this critical transition with confidence and clarity.

When a young adult with IDD turns 21, school-based services end and the transition to adult services begins. This is often one of the most challenging moments for families. Our team is experienced in this transition process and works directly with school districts, support coordinators, and DDD to ensure a smooth path to Day Hab.
