Position Overview: We are seeking a compassionate and dedicated Direct Support Professional (DSP) to provide care and support to individuals with disabilities or special needs in a day habilitation program. This role focuses on promoting independence, ensuring safety, and enhancing quality of life. Key Responsibilities: Provide direct care and support to individuals in a day habilitation setting Assist with daily living activities, including bathing, dressing, and meal preparation Encourage independence and participation in community-based activities Administer medications and monitor health conditions as required Maintain accurate and timely documentation of services and any changes in condition Advocate for individuals’ rights, well-being, and safety Collaborate with a multidisciplinary team to develop and implement individualized care plans Offer emotional support and companionship Qualifications: High school diploma or equivalent required Valid driver’s license and reliable transportation Ability to safely administer medication in accordance with established protocols Knowledge of patient care practices and basic first aid Additional Information: This job description is not all-inclusive. Full job description Position Overview We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to provide high-quality nursing care to individuals with intellectual and developmental disabilities (IDD) in a structured day program setting. The LPN plays a key role in supporting participants’ medical needs while promoting independence, dignity, and overall well-being. This position requires collaboration with an interdisciplinary team to ensure the delivery of person-centered, holistic care. Key Responsibilities Provide direct nursing care and health-related support in accordance with established care plans Administer medications and treatments as prescribed, ensuring compliance with all regulatory standards Monitor and document participants’ health status, reporting any changes promptly Maintain accurate, detailed, and timely clinical documentation Assist with medical evaluations, appointments, and coordination of care Support participants with activities of daily living (ADLs) as needed Promote independence and encourage safe participation in program and community-based activities Advocate for participant rights while ensuring a safe and respectful environment Collaborate with interdisciplinary team members to develop, implement, and update individualized care plans Provide health education and guidance to participants and their families Respond effectively to medical emergencies and ensure proper follow-up care Qualifications Active Licensed Practical Nurse (LPN) license in the state of employment, in good standing Prior experience working with individuals with intellectual and developmental disabilities preferred Working knowledge of conditions common in IDD populations (e.g., epilepsy, autism spectrum disorder) Current CPR/First Aid certification, or willingness to obtain Strong clinical, communication, and organizational skills Ability to work both independently and as part of a multidisciplinary team Why Join Us We are committed to fostering a supportive and collaborative work environment where employees can thrive professionally.
